An Insurance Services Office (ISO) standard form. It records that the insurer has approved an alarm or automatic sprinkler system at the described location and given a premium credit for it, and commits the insured to keep the system working and to report any change or removal.

What the insurer acknowledges. The system must be one the insurer has approved, and it must be at the location the policy insures.

When the insurer must be told. Any change to the system, including taking it out, has to be reported promptly so the insurer can review the credit.
