An ordinary form must be filed at least 45 days before use and is deemed approved after 45 days unless disapproved or review is extended for up to 45 additional days.

Applies to: Ordinary nonexempt form

Conditions and exceptions: The commissioner may exempt forms and the statute separately excepts surety specially rated inland marine and unique-risk forms.
