Form
The listed forms or transactions are excluded from the ordinary §33-1-501 filing and approval requirement.
Applies to: Surety bonds individualized forms reinsurance ocean marine foreign trade and out-of-state delivery
Conditions and exceptions: Apply the individualized-form and marine categories narrowly and consider the separate rules for domestic-insurer forms used outside Montana.
Form
A covered form must be filed and approved at least 60 days before delivery. It deems approved after 60 days without action but the clock is tolled while requested information is outstanding; the insurer must give a separate notice at least 10 days before market use when relying on the deemer.

Conditions and exceptions: Affirmative approval waives the remaining wait and the commissioner may grant a discretionary exemption.
Rate · Rule
All rates intended for use must be filed with supporting data. Current CSI guidance requires submission on or before the effective date and characterizes the method as file-and-use.
Applies to: Ordinary rates and rating rules
Conditions and exceptions: Reinsurance certain aviation-related rates ocean marine and surplus lines are outside the ordinary filing requirement; workers compensation has separate treatment.
Form · Rate · Rule
CSI expressly instructs that rate and/or rating-rule filings and form filings be submitted separately and requires the corresponding SERFF tracking numbers.
