Form

Method

A covered form must be filed and approved at least 60 days before delivery. It deems approved after 60 days without action but the clock is tolled while requested information is outstanding; the insurer must give a separate notice at least 10 days before market use when relying on the deemer.

Applies to: Ordinary policy forms certificates applications riders endorsements and renewal certificates
