Annual Filing

An annual SERFF Form Schedule and officer certification are due no later than July 1 for the listed products. Workers' compensation has a parallel July 1 statutory annual-report requirement for requested forms.

Conditions and exceptions: Regulation 5-1-18 excludes ERISA excess-loss forms from its annual report; that non-P&C product is not modeled here.

Attachment

The Form Schedule must identify each form by name number edition type action and required readability data. Ordinarily the actual forms are not attached; Regulation 5-1-18 requires attachment of the Colorado PPA summary disclosure form while its ERISA excess-loss exception is outside this P&C dataset.

Exemption

Colorado exempts qualifying exempt-commercial-policyholder business from specified rate filing approval and form certification requirements. Regulation 5-1-13 requires disclosures certifications and records for use of unfiled rates and forms.

Method

A private-passenger auto insurer must list and certify each new covered form at least 31 days before use. A commercial-auto policy with an individually-owned private-passenger-auto-type endorsement follows the same certification requirements.

Method

Unless a statute specifically requires prior approval Colorado classifies rate rule and loss-cost filings as file-and-use. Rates must be filed before distribution release to producers premium collection advertising or other use.

Colorado's current SERFF corpus exposes Form Rate Rule and Rate/Rule filing types but no combined form-rate-rule type. Product form filings and rate/rule filings therefore operate as separate submissions in current practice.
